Insights into a Multidisciplinary Approach to Medication Safety in Acute Care

Medication management is a complex multi-stage and multi-disciplinary process, involving doctors, pharmacists, nurses, and patients. Errors can occur at any stage from prescribing, dispensing, and administering, to recording and reporting. Interdisciplinary communication is essential to reduce medication errors. This professional development activity will provide strategies for optimizing safety when prescribing and administering medications in an acute care setting and when monitoring patients for adverse reactions.

Session Objectives:

  • Understand medication safety principles.
  • Identify the medications commonly involved in medication errors.
  • Describe the common break points leading to a medication safety event and best practices to prevent medication errors.
  • Understand the importance of medication reconciliation during transitions of care.
  • Identify the important disciplines involved in a medication safety program.

 Successful completion of learning activity:

Participants who view the module in its entirety, pass the post-test with 80% or higher and complete the evaluation form, will receive an electronic certificate via email. RNs completing the activity will receive a 1.0 contact hour of continuing education.
 

The Michigan Nurses Association (MNA) is accredited as a provider of nursing continuing professional development by the American Nurses Credentialing Center’s Commission on Accreditation.

AWARDING OF CE

  • Participants who achieve a minimum passing score of 80% will receive a certificate awarding 2.0 contact hours.
  • Participants who do not achieve a passing score will have the option to gain success with the material by repeating the material and test at no additional cost.
